Indicators and Patient JourneyEveryday practice consists of many patient contacts. The doctor, the nurse and the secretary have different functions in relation to the individual patient. The electronic health record is the common tool to document the efforts of everyday life, and thus ensuring the continuity of patient progress. To develop the quality it is important to set targets for the treatment of each patient. DAK-E has developed indicators to demonstrate the extent to which goals are achieved. These data are supplemented with electronic entries of additional information on each patient. It typically occurs in relation to the annual checkup. The indicators give the individual practice an overview of the number of patient journeys that the practice handles. There is a continuous feedback of data for all practices' patients within each disease area. This feedback is done via an encrypted site on sundhed.dk. The feedback provides an opportunity to check up on each patient's process. However, it also gives practices the opportunity to get an overview of the quality that the practice generally delivers on the various disease areas. So the feedback allows the practice to compare itself over time and with other practices' achievements, both within the municipality as well as with the region and nationwide.
